What Is Tadalafil 20 Mg?
Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor. It works by relaxing smooth muscle and increasing blood flow to specific tissues, which is why it is approved for erectile dysfunction and, under the brand Adcirca at different doses, for pulmonary arterial hypertension.
Tadalafil tablets come in four FDA-approved strengths: 2.5 mg, 5 mg, 10 mg, and 20 mg. The 20 mg strength is the maximum as-needed dose used for ED. It is not one of the standard daily-use strengths (those are 2.5 mg and 5 mg).
On-label vs. off-label use
- On-label at 20 mg: as-needed treatment of ED (10 mg starting dose, adjustable up to 20 mg or down to 5 mg based on response)
- Off-label / not standard: daily use of the 20 mg strength. Some published studies have looked at daily 20 mg dosing in specific populations, but this is not the FDA-approved daily regimen and should only be considered under direct medical supervision
What Is Tadalafil 20 Mg Used For?
Erectile dysfunction (ED)
This is the primary approved use of the 20 mg as-needed dose. Clinical trials have shown tadalafil improves erectile function compared with placebo, with an effect that can last up to 36 hours after dosing.
Benign prostatic hyperplasia (BPH)
Tadalafil is FDA-approved for BPH symptoms, but the approved BPH dose is 5 mg once daily, not 20 mg. Some studies have evaluated tadalafil at higher, less-frequent doses in men who have both ED and BPH symptoms, but this is a clinician-directed decision, not a standard label instruction.
Pulmonary arterial hypertension (PAH)
Tadalafil is also marketed as Adcirca for PAH, generally at a total daily dose around 40 mg (as smaller tablets), a distinct product and regimen from Cialis for ED/BPH. Adcirca and Cialis should not be taken together.

Who Should Not Take Tadalafil 20 Mg
Nitrate medications (such as nitroglycerin or isosorbide), including recreational nitrites (“poppers”). The combination can cause a dangerous drop in blood pressure.
Use caution if you:
- Take alpha-blockers (commonly prescribed for BPH or high blood pressure)
- Have a history of cardiovascular disease
- Have severe renal or hepatic impairment
- Are predisposed to priapism, or have a “crowded” optic disc or history of NAION
- Have a known hypersensitivity to tadalafil or Cialis
Common and Serious Side Effects
The most commonly reported side effect is headache. Other commonly reported effects include indigestion, back pain, muscle aches, flushing, and nasal congestion; these tend to be more frequent at higher doses.
An erection lasting more than 4 hours, sudden vision loss in one or both eyes, sudden decrease or loss of hearing, or symptoms of an allergic reaction (rash, hives, swelling of the face, lips, tongue, or throat, or difficulty breathing).
Drug and Lifestyle Interactions
- Nitrates: contraindicated, risk of severe hypotension
- Alpha-blockers: use with caution; may require dose adjustments and timing considerations
- Other PDE5 inhibitors (including Adcirca): should not be combined with tadalafil
- CYP3A4 inhibitors (such as ketoconazole or ritonavir): can increase tadalafil levels, requiring dose limits
- Alcohol: can contribute to lowered blood pressure and dizziness; discuss individual risk with a clinician or pharmacist
The Risk of Counterfeit or Unverified “Tadalafil” Products
Buying tadalafil from unverified online sellers or taking “natural” supplements marketed as a tadalafil alternative carries real risk.
